Ebenezer Integrated Care & Rehab offers Transitional Care, Long Term Care, and Memory Care. We focus on enhancing and accelerating recovery, managing long term medical conditions, and helping people regain the ability to function well in their day-to-day lives. As part of the Fairview Community Health and Wellness Hub, EICR enjoys opportunities for cooperative care and services. Our ultimate goal is to improve the health of those we serve while applying person-centered, culturally responsive approaches.

Covered parking is available in the 10th Street Ramp. Make sure to stop at the Hub's lobby front desk for a discount voucher! With the voucher, parking is $5.00.

Transitional Care

We offer short-term rehabilitation, including physical, occupational and speech therapies. Experts provide daily opportunities to improve the ability to function well in day-to-day life and accelerate recovery. Our program results in positive outcomes and a successful return home.

Long Term Care

Some medical conditions can be complicated and require specialized care. We’re here to provide daily support and ongoing medical attention to individuals with long term care needs. We consider each person’s physical, psychological, nutritional, and emotional requirements, and create a personalized care plan that aims to help them regain important skills for a better quality of life.

Memory Care

We are committed to helping older adults who are living with dementia make the most of their lives. Our self-contained, controlled-access unit offers nursing care, meals, social programs, exercise, and other services. Our specially trained memory care team members offer life-enriching activities and opportunities.
